How to Request an Appointment
Appointments can only be arranged by calling the office at (410) 571-9000.
When scheduling an appointment to see you pain management doctor, the receptionist will gather information to ensure enough time is allocated for your visit. We have the utmost respect for everyone's time, and we do everything we can to make the best use of both your time and our physician's time.
The following information will be needed in order to properly schedule your appointment:
Your date of birth
Home and alternate (work) phone numbers
Brief description of the reason for the appointment
Your primary physician or health care provider
Your insurance or HMO information
Once You Are Scheduled
Please arrive for your scheduled appointment on time. Patients who arrive more than 15 minutes after their scheduled time will be worked into the schedule and seen as soon as possible; but there may be a lengthy wait. If you are unable to keep an appointment, please call us at least 24 hours in advance so we may use those times for other patients. If you cancel an appointment with less than 24 hours notice, you will be charged a fee.
